That closeness is also the exposure, and it cuts both ways. A company that pays for an opinion has to be able to show the opinion was not bought. An expert who accepts the payment has to be able to show the same thing. Neither can show it alone.
Mature industries settled this a long time ago, and not by asking people to behave well. They settled it with structure: separate contracts, a single payer, a documented need, benchmarked fees, and a file that outlives the people who made it. Emerging medicine has the same relationship and almost none of the infrastructure.
